Overview

We are currently in search of an experienced Psychiatric Rehabilitation Direct Service Worker (PRP) for our Frederick, MD location. The Psychiatric Rehabilitation Direct Service Worker at Kolmac Integrated Behavioral Health and all its subsidiaries ("Kolmac" or the "Company") provides assessment and treatment services that promote recovery, full community integration and improved quality of life for persons who have been diagnosed with a serious and chronic mental health condition that seriously impairs their ability to lead meaningful lives. Direct Service Workers are required to provide collaborative, person directed and individualized services that focuses on helping individuals develop skills and access resources needed to reduce or eliminate the maladaptive behaviors that stem from a mental health diagnosis. 

Responsibilities

  • Conduct comprehensive initial assessments to evaluate client strengths, functional impairments, preferences, and needs.

  • Develop and implement individualized, patient-centered treatment plans with clear goals, objectives, and interventions.

  • Deliver services in clinic, home, and community settings, including:

    • Teaching and skills-based instruction

    • Encouragement of behavior change and personal growth

    • Skill modeling and practice

    • Support for wellness, self-management, and recovery

    • Assistance with self-care and social skill development

    • Coaching for independent living and self-sufficiency

    • Connection to academic, vocational, and employment resources

    • Facilitation of culturally relevant, relationship-building activities

  • Provide services through both in-person and telehealth platforms.

  • Complete clinical documentation, including treatment plans, progress notes, monthly summaries, and related paperwork.

  • Collaborate monthly with the client’s mental health therapist and interdisciplinary treatment team.

  • Maintain accurate and timely clinical records in accordance with CCG and regulatory standards, including electronic health record (EHR) documentation.

  • Participate in clinical team meetings, staff meetings, supervision, and required trainings.

  • Uphold client confidentiality and adhere to HIPAA and organizational compliance standards.

  • Communicate clearly and professionally with clients, families, and external partners.

  • Demonstrate strong time management, independent work skills, and ethical conduct.

  • Perform other related duties as assigned.

Qualifications

  • High school diploma.  Asociates degree + preferred. 
  • Previous experience working with emotionally and behaviorally challenged adults or experience working in a recovery-oriented program
  • Must be experienced and proficient in using an electronic health record system
  • Strong interpersonal skills
  • Ability to travel in the local area and perform field work
