Overview

We are currently in search of a Community Outreach Coordinator who will support our Western Maryland clinics in Frederick, Hagerstown and Cumberland, Maryland.  The Community Outreach Coordinator at Concerted Care Group (CCG) will play a vital role in bridging the gap between our organization and the communities we serve. This position focuses on fostering meaningful connections, building strategic partnerships, and increasing awareness of CCG’s comprehensive services.

 

As an advocate for our integrated behavioral health model, you will work closely with patients, families, referral sources, and community organizations to promote access to care and drive census growth. By developing innovative outreach strategies, delivering impactful presentations, and serving as a compassionate liaison, you will play a critical role in expanding our reach and ensuring seamless access to treatment for those in need.  

Responsibilities

  • Serve as a compassionate liaison to assist patients, families, and referral sources seeking treatment services.
  • Actively pursue new referral opportunities to drive regional growth and increase awareness of services offered by Concerted Care Group.
  • Serve as a liaison between referral sources and internal teams to address questions, resolve issues, and ensure seamless patient access.
  • Represent CCG at community events, health fairs, and public forums to engage community members and build trust in our programs.
  • Develop and deliver educational workshops, presentations, and materials to inform the community about behavioral health, substance use treatment, and wellness initiatives offered by CCG.
  • Build, maintain, and enhance strategic business partnerships with referral sources, including awareness organizations, probation and parole, first responders, hospitals, treatment centers, clinical and medical professionals.
  • Identify and establish strategic partnerships that align with CCG’s mission and expand access to care. Assess and develop strategies to improve patient access and drive census growth.
  • Collaborate with the marketing team to design and implement campaigns, including social media posts, email newsletters, and print materials, to reach target populations effectively.
  • Coordinate and execute outreach events, including health fairs, workshops, and information sessions, handling logistics such as venue selection, scheduling, and material preparation.
  • Monitor and evaluate the effectiveness of outreach initiatives, collecting data and preparing reports to identify areas for growth or improvement.
  • Assist in the development of community-based programs to address specific health and wellness needs identified through outreach efforts.
  • Support internal teams by providing insights into community needs and ensuring alignment between outreach activities and organizational goals.
  • Foster long-term partnerships with referral sources and providers, ensuring they are educated about Concerted Care Group’s services, treatment options, including Medication Assisted Treatment and admission processes.
  • Maintain an up-to-date understanding of CCG’s services, policies, and procedures to ensure accurate communication with community members.
  • Submit timely and accurate reports, including Salesforce data, expense reports, annual strategic plans, and monthly updates.
  • Demonstrate knowledge of and adherence to state and federal confidentiality regulations.
  • Participate in and complete all required training sessions, ensuring alignment with company policies and procedures.
  • Provide weekly schedules and regular updates to supervisors demonstrating a committment to excellence and success. 

Qualifications

Must Haves:

  • Strong understanding of analytics and the ability to leverage data for decision-making.
  • Excellent communication, negotiation, and relationship-building skills.
  • Business development experience, with the ability to manage and grow a territory.
  • Demonstrate the ability to be adaptable and solution oriented.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Valid driver’s license and reliable transportation.
  • High school diploma.

 

Pluses:

  • Knowledge of Medication-Assisted Treatment.
  • Bachelor's degree in Marketing, Business Administration, Healthcare Management, or a related field required.
  • Experience with outreach and business development in the substance abuse, behavioral health or mental health space.
  • Experience with Salesforce CRM
  • Knowledge of call center operations and best practices.
  • Familiarity with managed care systems, including provider networks, cost management strategies, reimbursement models, care coordination, and regulatory compliance.

The pay range for this position begins at $70,000 and then increases based on factors, such as but not limited to, geographical location, industry knowledge, relevant experience, licensure, education, and skill level.
